4-Personal Data Collection Method and Legal Reason
Your personal data can be collected automatically or non-automatically by the DOCTOR for legal reasons, with your specific explicit consent, if you accept the use of it for the performance of the health service provision relationship or for legal reasons based on our legitimate interests. However, as clearly stated in paragraph 3 of Article 6 of the Law, personal data related to health and sexual life, protection of public health, preventive medicine, medical diagnosis, treatment and care services, planning and management of health services and financing, are obliged to keep confidentiality. It can be processed by the persons under it or by authorized institutions and organizations without seeking the explicit consent of the person concerned.
5-Rights of Personal Data Owner
Pursuant to Article 11 of the Law, the person whose personal data is processed may apply to the DOCTOR and make requests regarding the following issues:

* If personal data has been processed, requesting information about it;
* Access and request personal health data;
* Learning the purpose of processing personal data and whether it is used by us in accordance with these purposes,
